For piercing or tattooing a minor, a parent or guardian must complete the consent form.
Minor forms must be notarized — no exceptions. A parent or legal guardian completes the form and has it notarized before the appointment. Both the minor and the parent/guardian show ID at the time of service. Don't have a notary? We do — book a notarization with us.
Pierced anywhere in Florida and it became infected? The state requires this report be sent to the health department. Even if you weren't pierced here, we'll gladly help you and submit it for you.
